Veterinary Check, Left Eye:
Patient was approached in enclosure to see if eye medication drops could be applied without restraint. Had to restrain patient in order to place medication.

Brief visual exam of left eye shows decreased anterior chamber size and partial collapse of the chamber. Ther is moderate, raised fibrous tissue present along central corneal laceration site.

Eye medication given.

Bird has appointment with veterinary oplthalmologist tomorrow for recheck and possible surgery, if indicated.

When going to give midday eye drops noticed OS was consistantly leaking fluid and feathers under that eye were wet. Grabbed and brought into hospital. OS was very soft and had significant gunk/drainage on the globe and on the lower lid. The eye had a bad odor like an infection. Used a sterile swab to remove the drainage. BNP ointment was applied to the globe and placed inside the lower lid.
OD has a scab present on the upper lid. It does appear to be tight and restricting some movement of the lid. However when manually moved the eye lids can fully close and the tird eye lid can close freely. The scab is dry and should fall off naturally.

NOLO and BARP. Handled to get a weight. OS is mostly crusted shut with a small gap laterally. There is a cloudy discharge that has accumulated in this area. Able to flush out with eye wash. Globe is still firm to the touch. 2 drops of Ofloxacin in the eye and then another we were able to lift the lower lid and place a drop in that seemed to move around the surface of the globe better. Placed back in enclosure and she opened the eye completely. It has a lot of discharge and is very cloudy. The lip of the lower lid seems to sag a little and the eye medially seems to twitch like it wants to blink but don't close completely.

Eye was crusted shut when I went out to treat this morning. Returned to grab. Lid has wet and dry gunk crusting lashes shut, swabbed off wet and applied a warm wet compress for ~5 min to soften crusted area. Swabbed as much as I could off but did have to gently pinch and pull most of it off of lashes. Applied drops and returned to enclosure - reperched. NOLO

Checked outside with the goal of NOT handling if not necessary.
The eye was open so did not handle. There is a very large thick scar/ fibrin plug over the laceration. My initial concern was that he might not be able to blink over the thick scar, but I used a Q-tip to stimulate a blink and he is able to close the lids and move the third eyelid over the cornea.
Placed eye drops. Since we didn't handle I didn't check bruising or SQ emphysema.
I made a video and will attach, as well as email to the ophthalmologists.
Continue all treatments. Recheck 4/26. Dr Gardner

Grabbed to rinse OS with sterile saline. Decent amount of gunk had built up on and inside lower eyelid. Gunk would not come off with the rinse or by gentle wipe with gauze. Did not want to irritate the eye further so stopped. After rinsing eye was able to open and close with more ease.

Grabbed for tx this morning and the central/lateral area is more open than last night. Consulted Dr Tracey & Dr G - potential appointment with an ophthalmologist upcoming, advised to remove sutures in the meantime. Brought inside and anaesthetized under 3% ISO and oxygen via mask. Intubated with 3.5mm ET tube and monitored breathing visually. Removed remaining three sutures from OS lid. Difficult to see - edge of lid is gunky and watery. Bird started coughing/gurgling around ET tube at the end; salivating; extubated and masked with O2 while she woke up. Admin 0.14 torb IM. Will feed medicated food at midday.

Anesthetized for radiographs. Exam was after radiographs.
On PE - OS appears to be deflated at this point and I can see fluid actively leaking from lateral canthus of eye. Suture appears to have been lost from the lateral aspect of the lid.
We did a DV radiograph of the head prior to this and the eye appears to be inflated. I wonder if we dislodged a fibrin clot durin anesthesia.
The bird was almost awake so I did not intervene. My thought is that the cornea could be sutured, but I have never done it. I have reached out to ophthalmologists I know regarding technique.
I may be able to come by Monday and do it, or Friday afternoon (but that may be too long).

BAR, Perched. Bird was a little feisty but normal behavior. NOLO. Sprayed with Bronco for lice.

Preparation for Left Eye Evisceration Surgery: CANCELLED SURGERY due to refilling of anterior chamber.

Patient was captured and placed under general Isoflurane anesthesia and intubated with medium sized Cole ET tube.

Examination of the left eye showed normal shape and refilling of the anterior chamber. There grossly did not appear to be any active aqueous leakage and the longitudinal defect in the cornea has sealed shut, at least superficially.

It was discussed and decided to give this eye time to see if the cornea will heal completely in order to preserve the left eye.

A temporary tarsorhapy was performed to protect the cornea. Simple interrupted sutures were placed with 4-0 PDS to appose the upper and lower eyelids. Suturing was performed to prevent the suture material from touching the corneal surface. The medial canthus was left open a bit so opthalmic eye drop medication can be placed in the medial canthus.

Plan:
Leave temporary sutures in place for approximately 10-14 days.

Change eye medications to non-steroidal drops and antibiotic drops, once daily.

D/C tramadol and continue meloxicam systemically.

Veterinary Exam, Check Left Eye, 4/10/24:

Patient was manually restrained for exam.

Large blood clot present on central surface of the left globe. When this was gently removed with swab, there became evident a very large tear in the cornea and aqueous fluid immediately leaked from anterior chamber and globe collapsed.

This left eye will not be savable and evisceration is recommended since owl.

Found more scab on the lateral side of the TMT up to the hock. Lightly scrubbed above and below both eyes with chlorahex swabs. Stained both eyes and both were neg. Opted to move out to R32 and test tomorrow for HPAI so we can use her as a protected contact "mom". SQ air is still present
